How to Refer a Patient
- Email us for a new clinic registration and patient referral form.
- Complete both forms taking care to include relevant case history and laboratory results on the patient referral form.
- Direct patients to our website and our booking page.
- Following the consult with the patient, all consult notes will be emailed to you, along with the proposed nutritional management strategy and recommended follow up with the treating veterinarian.
